Here at ICARE, we ask every
Wisconsin triad member to register with our Wisconsin Adoptee - Birth
Family Registry. In the event we are able to facilitate a match within the
registrations already on file, it will save you time
and financial expense. If we do not have a match for you, we suggest the
following for your information.

To
Searching Adoptees without Birth name:
We suggest that you contact the State
of Wisconsin Search Program. When doing so, you will receive a
Search Packet that must be filled out and returned by you. The
packet will detail your options, whether it is just requesting
your Medical/Genetic/and Birth family Social History or asking the
State to actually do your Search and contact your Birth Mother to
see if she is receptive to having contact with you.
There is a fee for
their service. Be sure and ask up front what will be required of
you so that you will know what to anticipate. You may qualify for
a reduced rate based on need. Please inquire. You can make your
initial request for a Search Packet to:
Include your adoptive
name, DOB, Adoptive parents names, place of birth, a current
phone number, your current address, and the Agency that handled
your Adoption, if known. Tell them ICARE referred you to them.
Should they complete your search for you, let us know the results
so we can post accordingly. Good Luck!

To
Birth Family Members/BirthParents and Adoptees with Birth name:
ICARE Search help is
available to you by request. Our Search Staff does not ask for an
hourly fee. However, reimbursement of expenses is expected. A
search can be done in a range from "Free to maximum
$300", (Depending on County of Birth
and expenses involved). Each case will be evaluated in advance and
quoted for your pre-approval.
(The State
of Wisconsin Search Program at this time has no provision
within it's Adoption Law to
Service Birth Parents or Birth Family Members.)

Searching Adoptees born at the Martha Washington Home for Unwed
Mothers AKA Booth Memorial Hospital located at 6306 Cedar Street,
Wauwatosa, WI.
You need not contact the State Search Program. You may contact the
Salvation Army direct at:

This Facility works with the
Salvation Armys Missing Persons/Adoption Reuniting
Organization, implementing the Law via the the established
guidelines of the State Search Program. The Home mentioned above
was run by the Salvation Army. All the birthing records are housed
in Des Plaines, IL.
You will need to submit a notarized
letter of request for their services. There is an Administration
fee of $50 to process your request for them to contact your Birth
Mother to see if she is receptive. If she is willing to sign an
Affidavit of Consent, they will reunite you. This avenue of Search
is the most economical financially. Please dont delay. Contact
them today and Request their Services. Tell them ICARE referred
you to them.
The only other birthing facility in
Wauwatosa was "Milwaukee County General Hospital". they
can tell you if you were NOT born at the Salvation Army Facility
described above and then you can contact the State Search Program
for service. Often the Hospital or Institution line was left
blank. This is a "good" indication the Home mentioned
above is the correct birthing place. Let us know should they
complete your Search so we can post accordingly. |
